I have the book and disc in excellent shape. My copy also includes a foam disc pad, 12" dia, with a paper note "Caution - The enclosed pad is designed for use on transcription and 12 inch Hi-Fidelity custom turntables. It may also be used on all turntables in manual operation. Please trim pad to fit your particular turntable. Remove pad when using 45 RPM adaptor." Then there is a small RCA logo with dog, and the code: LM-1802
